The Self-Management Programs Network allows patients, caregivers, and healthcare providers to access information about self-management programs in Southwestern Ontario. Programs are provided by Southwest Self-Management, Waterloo-Wellington Self-Management, Hamilton Niagara Haldimand Brant Self-Management, and Erie-St. Clair Self-Management.

What are self-management programs?

 

Self-management programs are designed to provide you with tools to help you confidently manage your health condition and improve your quality of life, and how to support someone you care about with their health condition. Healthcare Providers also have access to workshops that promote working in partnership with patients and caregivers. Programming is free and available for anyone 18 years and older in Southwestern Ontario.
