The Self-Management Programs Network allows patients, caregivers, and healthcare providers to access information about self-management programs in Southwestern Ontario. Programs are provided by Southwest Self-Management, Waterloo-Wellington Self-Management, Hamilton Niagara Haldimand Brant Self-Management, and Erie-St. Clair Self-Management.
What are self-management programs?
Self-management programs are designed to provide you with tools to help you confidently manage your health condition and improve your quality of life, and how to support someone you care about with their health condition. Healthcare Providers also have access to workshops that promote working in partnership with patients and caregivers. Programming is free and available for anyone 18 years and older in Southwestern Ontario.
A 4.5 hour virtual workshop for clinicians and non- clinical support staff members to improve verbal and non-verbal communication skills to positively impact health outcomes for patients and their families
The Choices and Changes webinar provides clinicians with an opportunity to explore their own beliefs about the change process and hear some of the research-informed premises around behavior change.
During the webinar participants will review:
Research in regards to health behaviour change and the premises of change
Review a model for change

Brief Action Planning (BAP) is based on the premise that motivation comes within the person. The clinician's role is to guide people toward creating their own plans, adjusting support as needed, and always seeking to build confidence in those they work with.
People often think about wellness in terms of physical health - nutrition, exercise, weight management, etc.., but it is so much more. Wellness is a holistic integration of mental, physical, and spiritual well-being, fueling the body, engaging the mind, and nurturing the spirit.
Caring for a family member or friend with a health condition can be physically, emotionally and financially challenging. This program builds the skills caregivers need to take better care of themselves as they provide care for others.
